요약

이비인후과 영역에서 만성 부비동염이 차지 하는 비중은 대단히 크며, 외과적 치료로서는 최근에까지 근치수술의 대명사격인 Caldwell-Luc법을 이용하는 것이 주류였다고 말할 수 있다. 근래에 와서 경제적 발전으로 인한 생활 향상, 의료보험제도의 확립, 그리고 단백효소 제 및 항생제의 발달 등으로 세균감염에 의한 중증형 부비동염은 점차 감소되어지고 그 대신 비 알레르기가 증가되고 있다. 이와같이 만성 부비동염의 경증화, 내시경의 진보 및 보급, CT scan에 의한 비강 및 부비등의 해부학적 정보와 병태의 명확한 파악 가능, 그리고 부비 강 점막의 질병에 대한 회복능력(reversibility) 둥으로 근치수술이 감소되고 있으며 심지어는 부비동염 근치수술에 대한 재평가를 고려해야 된다는 사태를 야기하기에 이르렀다^4,19).
