요약

기관 피부 누공은 기관절개술의 합병증으로 그 빈도 는 3.3%에서 29%로 보고되고 있다.¹⁾ 이러한 누공은 피부의 상피가 기관절개창을 따라 기관의 점막까지 성 장하는 것으로 기관절개창의 폐쇄을 방해하는 것이다. 누공의 빈도는 기관절개술을 통한 기관 캔뉼(cannule) 의 유치 기간과 밀접한 연관성이 있다. Kulber 등²⁾은 기관캔뉼 유치 기간이 16주 미만인 경우에는 누공이 발 생하지 않으나 16주 이상인 경우에 70%까지 발생율이 증가한다고 하였다. 그 외에도 두경부 수술(특히 보존 적 후두절제술)시에 기관개창술(tracheal fenestration) 등을 시행하기도 하는데 이러한 경우에 발생한 기관피 부누공의 폐쇄가 어려운 경우를 간혹 경험하게 된다.
