중이염치료에 있어서 항생제의 선택

Bacterial Pathogens은 Acute OM 예의 70% 에서 그리고 recurrent or chr. OM c effusion 예의 50% 에서 Culture되며 predominant organism 으로는 Streptococcus pneumoniae (24〜52%). Hemophilus influenzae (11 〜25%). Branhamella Catarrhalis (1〜19%) 등돌 들수 있는데 이 중에서도 Streptococcus Pneumoniae와 Hemophilus influenzae에 대해서는 Amoxicillin (40mg/kg/day #3)이 drug of choice이나 Hemophilus influenzae의 30%， Branhamella Catarrhalis의 75%이상이 β-lactamase를 생산하므로 Amoxicillin에 ineffect시 에는 β-lactamase producing Strains에 effective한 Clavulanate potassium (Clavulanic Acid : 10mg/kg/day #3)이 첨가된 Augumentin (40mg/kg/day #3)으로 치료하는데 3% 에서 Watery diarrhea를 경험하며 그외 cefaclor (40 rag/kg/day #3)，EM(50mg/kg/day #4)+Sulfisoxazole (150mg/kg/day #4), Trimethoprim(8 mg/kg/day #2)-Sulfamethoxazole (40ing/kg/ day #2)중 cefaclor(ceclor)을 복용한 children 의 1〜2% 에서 serum sickness like illness (fever, joint pain, polyarthralgia) 외 Erythema multiforme 를 경 험 하며 EM + Sulfisoxazole 인 Pediazole은 Hemophilus에 대해 highly synergistic effect이나 Pneumococci에 대해선 antagonistic effect 이며 vomiting과 upper abdominal pain이 EM의 비교적 common side effect 이고 severe rashes와 epidermal necrolysis는 대개 Sulfonamides와 유관하다.
