전두개저 및 중두개저의 중앙부분에 발생한 두개외 또는 두개내 병변에 있어서 내시경적 비내접근법은 두 개저 수술에 있어 서 가장 중요한 무기 가 되었다. 이러한 접 근법은 최소침습적이고, 환자가 잘 견뎌낼 수 있으며, 특히 전두개저 및 터키안 병변의 노출 및 치료에 효과적일 수 있다. 그러나 측두개저의 병변에 대해서는 지금까지도 고 전적인 외부접근법이 시도되고 있는 실정이다. 이비인후 과에서 흔히 접할 수 있는 비인강 혈관섬유종의 경우, 비 인강, 비강, 사골동 및 접형동에 국한된 경우에는 비내접 근에 의한 내시경 수술로 어렵지 않게 제거될 수 있으나, 종양이 익돌구개와(pterygopalatine fossa) 및 즉두하와 (infraemporal fossa) 등 외측으로 진행되는 경우를 드물 지 않게 접할 수 있기에 이에 대한 대처 또한 필요하다 (Fig. 1).1) 이에 저자들은 측두개저, 그중에서도 특히 익 돌구개와 및 측두하와로의 내시경적 비내접근법에 대해 임상 경험을 바탕으로 문헌 고찰과 함께 분석하고자 하 였다.
